	After completing and submitting this form you will be redirected to the forum agreement page, read it, click the I agree link if you agree then fill out the forum registration using your ISP business name as your forum user name. I will verify the information provided here then approve your forum membership. This usually takes only a few minutes if I am in the office at the computer. If I am out of the office or traveling it may take up to 24 hrs for this process to be completed.
	Only webmaster@your-website-name.com or admin@your-website-name.com email addresses will be accepted for signup using this form. This is to verify your ISP ownership. I will send an email to the webmaster or admin email address you provide, you must then send a reply. You can use an email address different then webmaster or admin for the forum if you like. All fields except notes are required, if you are going to leave anything out don't waste your time because your submission will be deleted upon receipt. Your mom will then be called to discuss how many brain cells you may actually have.
